Student Evaluation Questionnaires

Student questionnaires are a crucial part of the evaluation of our program. In any course in which a historical project could be used, regardless of whether a project is actually used or not, we kindly request testers to administer student questionnaires. Please administer our anonymous questionnaires twice, once at the beginning and once at the end of the course. When handing out the questionnaires at the beginning of the course, ask each student to write a 4 to 7 digit code at the top of the form, keep the code in a memorable place, and then use the same code when completing the questionnaires at the end of the course. No names are to be written on the questionnaires. The details for administering and returning the questionnaires and cover sheet are as follows.
